Primary care providers play a major role in supporting people in their local community or region – keeping them healthy, and treating them when they’re sick.
But who is helping the GPs, the Allied Health Providers, the population in delivering that care? Who is helping connect it all together and bring it into context, to ultimately ensure the community as a whole is healthier and connected?
In Australia, often much of this is managed by Primary Health Networks (PHNs).
